Job Description:
A company is looking for a Clinical Case Manager Behavioral
Health DSNP.
Key Responsibilities
Conduct comprehensive assessments of members' needs and eligibility
for services
Utilize clinical judgment to develop care plans and coordinate
services for members
Provide crisis intervention and follow-up care to ensure members
receive appropriate treatment
Required Qualifications
3 years of direct clinical practice experience post-master's
degree
3 years of knowledge in mental health and substance abuse
disorders
3 years of experience using Motivational Interviewing and
technology skills
Valid unrestricted independent professional behavioral health
clinical license
Must be available to work Monday through Friday, 8:00am to
5:00pm
